Social Prescribing Service

Social Prescibers offer a service to connect people to activities, groups and support that improve wellbeing.

If you live in North Yorkshire the council offers a service via Living Well (https://www.northyorks.gov.uk/adult-care/living-well-north-yorkshire) but there are some patients who are not eligible to be referred to them. For these patients and our patients who live in West Yorkshire we have a Social Prescriber who is employed by our Primary Care Network. Karen has been working locally for a number of years now so you may have met her around Tadcaster.

To be referred to a Social Prescriber please contact our reception team or mention you are interested when you are next seen by the GP or Nurse.

The website below has more information on how Social Prescribing works for patients accross England.
